Medication Administration

Any over the counter medication (Cough Drops, Tums, Tylenol, etc.) and prescription medication require a doctors order. Students will not be given prescription and/or over-the-counter medication without a completed NEW Medication Authorization Form  signed by the parent and students physician. All medication MUST be brought to school in the original container and labeled with the student's name. A reminder will be sent out at the end of the school year to all parents whose child may have taken medication during the school year. At the end of the school year, any medication not picked up by the parent will be disposed of. All emergency medication (i.e. inhaler, Epi-Pen, diabetes supplies) will be sent home with the student at the end of the year.

Asthma medication and administration

Students with asthma have the option to self-carry their inhalers while in school, at school sponsored activity, before or after normal school activities. A Student Self-Administration Consent Form is required. Please attach a copy of the original prescription label to the consent form. A physician signature is not required, only the parent/guardian signature is required in the State of Illinois. Public Act 096-1460 .

Please note: Illinois Public Act 099-0843 requires schools to request from the parent or guardian of students with asthma an Asthma Action Plan (AAP) The Asthma Action Plan can be filled out by the student's primary care provider and submitted to the school nurse.

Epi-Pen

Students with severe allergies and asthma may require an Epi-Pen emergency auto-injector for anaphylaxis. Students and parents have the option to self-carry their Epi-Pen while in school, while at school sponsored activities, or before or after normal school activities. Students may also keep their Epi-Pen in the main health office. Written authorization is required from the student's parent/guardian and student's physician. Please fill out the Illinois Allergy Emergency Action Plan  or a Medication Authorization Form if your child has a severe allergy and requires emergency medication. Public Act 096-1460 .

 PUBLIC ACT 096-1460

A school, whether public or nonpublic, must permit the self-administration of medication by a pupil with asthma or the use of an epinephrine auto-injector by the pupil, provided that:

1. The parents or guardians of the pupil provide to the school written authorization from the parents of guardians for the self-administration of medication or for the use of an epinephrine auto-injector, written authorization from the pupil's physician, physician assistant, or advanced practice registered nurse; and
2. The parents or guardians of the pupil provide to the school the prescription label, which must contain the name of the medication, the prescribed dosage, and the time at which or circumstances under which the medication is to be administered, or for use of an epinephrine auto-injector, a written statement from the pupil's physician, physician assistant, or advanced practice registered nurse. The information provided shall be kept on file in the office of the school nurse or, in the absence of a school nurse, the school administrator.
